The following surveys are designed to help determine the cause of your
discomfort. Each survey in an extensive list of symptoms you may have
experienced. Please be as honest as possible. You may have the results
submitted to the Lebro Center for further analysis by Dr. R Tony Lebro.
If you wish, the doctor will contact you by phone to discuss your results
and possible remedies.
Privacy:
The information provided within the following questionnaires is treated
with the strictest confidence. Your communication with the Lebro Center
is absolutely considered private and no portion of your personal information
will be shared with any other party.

This questionnaire is designed to aid the practitioner in assessing a patient's potential need for a Clinical Purification Program.
Symptom Survey
This health questionnaire will help to isolate vitamin deficiencies. From this survey, Dr. Lebro can create a personal profile, including a list of herbal remedies specifically tailored to your needs.
Instructions:

Clinical Purification Program:
Regular nutrition and supplemental nutrition in the form of megavitamin
and mineral dosages and extra quantities of oil are used to help
the body flush out toxins and repair and rebuild the areas that
have been affected by drugs or other toxic residuals.
